Youth Ministry Registration

I give permission for my child to attend St. Anne’s Episcopal Church program and to be photographed, videotaped, or audiotaped. I further agree that any or all the materials recorded may be used, in any form, as part of any future production(s) made by and for St. Anne’s Episcopal Church, and further that such use shall be without payment of fees, royalties, special credit, or other compensation. If recorded materials are used in St. Anne’s web site, parish newsletter, or Diocesan newspaper the name of the participant will not identified.
