Nametag Order Form

You may order a new nametag at no additional cost to you by mailing/delivering the incorrect nametag to the Parish House (199 Duke of Gloucester Street) with a note explaining what needs to be updated. You are eligible for this if:

You have a pacemaker or other medical device that prevents you from wearing a magnet on your chest, and/or
Your name is misspelled.

NB. If you are ordering a replacement, there is no need to fill out the form below.

If you did not order a nametag but would like one, please use the form below to order a new nametag. The cost is $20 per nametag. Payment should be sent to the Parish House in the form of a check made out to “St. Anne’s Parish” with the memo “Nametag”. Your order will be processed upon receipt of the check.
